After a total knee replacement (TKR), many patients are surprised by how quickly their physiotherapist encourages them to start bending and straightening the knee. This focus on early movement is not about rushing recovery; it is grounded in strong scientific evidence showing that early, structured physiotherapy is one of the most important factors influencing long-term outcomes.
The role of early movement
Regaining knee range of motion is the first priority after surgery. Both bending (flexion) and straightening (extension) are essential for everyday activities such as walking, standing from a chair, and using stairs comfortably.
Research shows that knee flexion improves most within the first three months after surgery, while extension can continue to improve for up to six months (Mutsuzaki et al., 2017). This early period is therefore a critical window. Consistent physiotherapy and regular exercise during this time can make a meaningful difference to long-term comfort and function.
Why timing matters
When rehabilitation begins promptly, most patients make steady and predictable progress. Delays, however, can make regaining full movement more difficult. Clinical experience supports what the research shows: the body responds best when movement is encouraged early and progressed gradually.
While improvement is still possible later, studies indicate that gains in knee movement tend to plateau after around twelve months (Nerhus et al., 2010). This highlights the importance of making the most of the early stages of recovery.
Protecting the rest of your body
Limited knee movement does not only affect the knee itself. When the joint does not move freely, other areas such as the ankle, hip and lower back often compensate. Over time, this can lead to discomfort, imbalance or secondary problems elsewhere in the body.
Achieving good range of motion early helps restore more natural movement patterns and reduces unnecessary strain on surrounding joints.
Patience, effort and consistency
Recovery after knee replacement is rarely linear. Some days feel easier than others, and temporary stiffness is common. What matters most is steady effort over time, guided by professional support.
Physiotherapy provides structure, reassurance and progression, helping patients move safely through each phase of recovery with confidence.
